エキサイト翻訳を使用しましたが機械翻訳なので理解できませんでした。

Unfortunately, our records show we still have not received the full
documentation required prior to making any futher commission payments.
Before sending payment, we must receive the following for your
"japandreamsuccess" account:

1. Signed last page of the Independent Affiliate Agreement Terms and
Conditions

2. Signed last page of the Independent Affiliate Policies and
Procedures

3. Clear copy of your government-issued photo identification (driver's
license or passport)


In addition to the 3 items above, we must also receive 1 of the
following depending on your residency location:

> IRS Form W-9 (USA residents only)

> IRS Form W-8 (All Non-USA residents only)


All items, along with helpful information for completing them, are
easily accessible from within your account. Simply log into your
"japandreamsuccess" account at http://website.ws/members, then click the
"Documentation" link in the left-hand navigation bar. You may return
the documents to us as scanned attachments in reply to this email, by
fax, or by mail. The specific fax number and mailing address are
listed in the same "Documentation" section within your account.

It is vital you return the required documents in full at your earliest
convenience. If your paperwork has not been received in time for us to
process it by the last business day of this month, your commission
payment will be held until the next month when we do receive your
proper documentation.
